APPOINTMENT

At the Animal Rehabilitation Center of Michigan, we understand that rehabilitation therapy is a collaborative effort between the pet, his or her owner, the referring veterinarian, and the rehabilitation practitioner. We do our best to make sure that everyone stays informed of the pet’s progress throughout the rehabilitation process.

Prior to your client arriving at their consultation, we ask that you complete the online referral form. If you are having trouble submitting this form, please contact us. We’d be happy to email you a hard copy. Please print it out, fill it in, and fax it to us at (248) 363-1138.

In order for us to evaluate your client completely, we ask that he or she bring any diagnostic tests that have been performed to the consultation. These may include X-rays, surgical reports, ultrasound or MRI reports, and so on. This information can also be sent to us directly via fax at (248) 363-1138 or email at rehab@unionlakepets.com.

Please note: Because we value the relationship we have with our referring veterinarians, our clients are always directed back to you for any medications, diagnostic tests, re-check examinations, or illnesses that may arise during the course of treatment. You are very important to us!
